CONTRACT / SCOPE OF WORK
Scope of Services
This agreement includes:
Submission of up to 2 insurance panel applications including follow up until completed 
CAQH profile setup or optimization
Basic EHR setup and up to 2 training sessions
Any services outside of this scope will be quoted separately.

Client Responsibilities
The client agrees to:
Provide all required documents in a timely manner
Respond to communication within a reasonable timeframe
Ensure accuracy of personal and professional information
Delays in response may impact completion timelines.

Timeline
Timeline varies based on responsiveness of the client and insurance companies
No guarantee is made regarding insurance panel approval timelines

EHR Training Boundaries
Includes up to 2 sessions
Additional sessions are billed separately
Ongoing tech support is not included unless agreed upon

Payment Terms
Payment must be made according to selected plan
Work begins after initial payment is received
Final deliverables submitted after final payment

Refund Policy: Due to the nature of service-based work: Payments are non-refundable once work has begun
